Leaving Japan on National Health Insurance: Cancellation and Final Settlement

By JP Tax Guide • • 3 min read

If you are enrolled in municipal National Health Insurance when you move abroad, include the insurance counter in your departure preparations. The practical task is to confirm when eligibility ends, how the account is recalculated and where later correspondence will go. This guide concerns municipal National Health Insurance; an employer’s insurance or voluntary continuation has its own procedures.

Follow your municipality’s departure process

Yokohama’s overseas-departure FAQ instructs residents to complete the resident moving-out procedure first and then visit the insurance counter. It calls for identification and any issued eligibility verification document. When all insured household members leave, the premium settlement is handled at notification.

Treat this as a concrete municipal example, then obtain the instructions for your own city or ward. Tell the office whether the entire household is moving or some people are staying. That distinction can affect what information the counter needs and whose insurance account is being discussed.

Ask for the effective date in writing

Write down the recorded overseas move date and ask the insurer to confirm the last day on which the coverage applies. Do not infer it from the day you visited the counter, the day a bill arrives or the day an eligibility document is returned.

Yokohama’s coverage-loss and benefit-repayment explanation distinguishes overseas and domestic moves: for an overseas move, loss of eligibility is the following day. It also explains that premiums and benefits paid for medical treatment are reconciled separately.

Separate billing installments from coverage months

Bring the latest premium notice and payment records. Ask the counter to identify the total recalculated charge, the amount already paid and any balance or refund. This is clearer than asking whether you can ignore "next month’s bill," because a bill’s due date alone does not identify the coverage period it settles.

For example, your worksheet might show a confirmed total charge, several payments already made and a remaining balance. The balance is the difference between those figures; it is not a new premium estimate based on the number of days until the flight. Use the municipality’s calculation rather than dividing an annual figure by 365 yourself.

Do not use former coverage after it ends

The Yokohama explanation above warns that insurance-funded medical costs may have to be repaid when coverage is used after eligibility has ended. A premium refund and a demand to repay a medical benefit are separate matters, so receiving one does not rule out the other.

If treatment falls near the move date, tell the clinic that your eligibility is changing and confirm the applicable coverage. Save receipts and any insurer instructions. If an incorrect insurer was billed, ask the relevant insurers how to resolve it instead of assuming a later premium adjustment will correct the medical claim.

Leave a clear settlement record

Before leaving the counter, check these points:

Keep the answers with your departure checklist. National Health Insurance settlement does not submit a pension withdrawal application; use the separate pension withdrawal guide when organizing that decision and its documents.
